'Exercise, sleep, screens' - World Health Organization issues first-ever guidelines for children under 5

Active kids are healthier kids

On Wednesday, the United Nations' public health agencyreleased its first guidelines on physical activity, sedentary behavior and sleep for children under 5.

"Improving physical activity, reducing sedentary time and ensuring quality sleep in young children will improve their physical, mental health and wellbeing, and help prevent childhood obesity and associated diseases later in life", said Fiona Bull, programme manager for surveillance and population-based prevention of noncommunicable diseases, at WHO.

Instead, "when sedentary, engaging in reading and storytelling with a caregiver is encouraged", the organisation said of young children.

Children between one and four years old should spend at least three hours in a variety of physical activities spread throughout the day, it said.

"What we really need to do is bring back play for children", said WHO's Dr. Juana Willumsen, who specializes in childhood obesity and physical activity. The new guidelines on physical activity, sedentary behaviour and sleep for children under 5 years of age were developed by a World Health Organization panel of experts.

Professor Andrew Przybylski, from the University of Oxford, said: "On the basis of common sense numerous recommendations make sense but in many ways the conclusions drawn about screens are out of step with scientific evidence of harm". The guidelines varied by age group: infants younger than 1, children 1 to 2, and those 3 to 4.

Issuing its first such guidelines, the World Health Organization has recommended the limit for two-to four-year-olds is an hour a day - or less is better. For babies who are not yet mobile, they should spend at least 30 minutes a day in prone position, or "tummy time", spread throughout the day while awake. Screen time is not recommended for 1-year-olds, and the guidelines recommend that it be limited to one hour a day for 2-year-olds.

The global guidelines recommend a limit of just one hour sedentary screen time, which includes sitting still playing games on devices, for those aged between two and five years old. Children aged one to two years old should spend at least 180 minutes being active and 11 to 14 hours sleeping, the report states. No more than an hour a day is the official guideline, while physical activity should be more vigorous still. In addition, they need better quality sleep and more time for active play in order to grow up healthy. Infants shouldn't be restrained for more than one hour at a time in strollers, high chairs or strap-on carriers.

"It induces a real conflict", said University of MI pediatrician Jenny Radesky, author of screen-time guidelines for the American Academy of Pediatrics in 2016.

Tim Smith of Centre for Brain and Cognitive Development, Birbeck, University of London described that distinction as "an oversimplification of the many ways young children and their families engage with screen media". "Replacing restrained or sedentary screen time with more moderate to vigorous intensity physical activity, while preserving sufficient sleep, can provide additional health benefits".
